An almost 2-year-old child died in a head on collision on Route 138 in Taunton on Saturday evening, the Bristol County DA reported.

At about 7:10 p.m., local EMS received a 911 calls reporting a car crash near 400 Broadway, along a section of Route 138 through Taunton, the DA’s office stated.

At the scene, first responders found a head on collision involving a white Chevy van and blue Toyota Rav 4 SUV. The van was located “centered in the northbound lane in the correct direction of travel,” the DA said.

The blue SUV appeared to have crashed into the van head on in the northbound lane, the release reported. Both vehicles sustained “heavy crash damage.”

Four adult men were traveling in the van, remained on the scene and did not display any injuries, the DA’s office said.

The other car was carrying a 25-year-old woman driving and her two young children, the DA stated. One child, an almost 2-years-old girl, was taken to a local hospital and pronounced deceased shortly after arrival.

The 5-year-old girl was taken to an out-of-state trauma center and was in stable condition, according to the report. The mother was also in stable condition and taken to Rhode Island Hospital.

Massachusetts State Police with the Bristol DA’s office and Taunton Police are actively investigating the incident.
